Answering YES to question 1 indicates that an individual may be symptomatic of COVID-19 and should be advised to self-isolate and use testing options available. The individual may not be eligible for treatment at this time. Answering YES to question 2 indicates the individual may be required to self-isolate and may not be eligible for treatment depending on the required length of self-isolation.
